Right Choice Training's ICAM Lead Investigator Training Course Program is designed specifically for anyone who will be leading an incident investigation team, which may include supervisors, safety representatives, managers, safety officers, union officials and inspectors etc.
The Incident Cause Analysis Method (ICAM) is a well-known and highly respected incident investigation methodology that has been used globally in many industries for over two decades. The aim of an incident investigation using ICAM is not only to find out what happened, but importantly, identify why it happened and how we can stop it happening again.
For those personnel appointed as Lead Incident Investigators, it is critical that they have in-depth knowledge and skills not only of how to investigate incidents, but how to manage resources to achieve the incident aim of prevention of recurrence and reduction of risk.

ICAM Training Course Overview 

ICAM (Incident Cause Analysis Method) is an internationally renowned, best practice systemic safety investigation analysis method. It aims to identify both local factors and failures within the broader organisation and productive system that contributed to the incident, such as communication, training, operating procedures, incompatible goals, change management, organisational culture and equipment. Through the analysis of this information, ICAM provides the ability to identify what really went wrong and to make recommendations on necessary remedial actions to reduce risk and build error-tolerant defences against future incidents. The ICAM process incorporates best practice Human Factors and Risk Management principles. ICAM methodology is used extensively by many of Australia’s leading industrial and mining organisations and throughout the world.

Learners will be trained in ICAM Analysis and BSBWHS515 - Lead initial response to and investigate WHS incidents. These unit describes the performance outcomes, skills and knowledge required to plan, conduct and report on investigations of incidents that have resulted in, or have the potential to result in, injury, damage or commercial losses of any kind..

This course is designed to cover incident investigation concepts including:

  • Initial response to incident
  • Causation models theory
  • 5 ‘Whys’ and Incident trees
  • Timeline
  • PEEPO Data Collection chart (People, Equipment, Environment, Procedures and Organisation)
  • ICAM Analysis
  • ICAM Report Preparations and Communication

Course Outline 

The key areas covered by this course include:

  • The legal requirements applicable to WHS accident and incident investigation
  • The legal requirements applicable to WHS accident and incident investigation
  • Incident notification and reporting
  • Identifying causative factors for accidents and incidents using accident causation models
  • Interviewing witnesses and/or take statements
  • Investigating accidents and incidents using ICAM Analysis
  • Develop and communicate ICAM investigation report

Course Inclusions

Australia’s first self-paced eLearning course is designed for those who wish to develop their skills in leading WHS incident investigations.
Learners will be provided online access to our state-of-the-art LMS system to complete the course.
Learners will be given access to our simulated business case study and incident scenario to lead the ICAM investigation in a simulated pathway. In addition, learners will be provided ICAM Investigation report template and Investigation quality checklist.

Certification

Learners who are deemed competent will receive a Certificate of Achievement of ICAM Lead Investigator Course and the Statement of Attainment of BSBWHS515 - Lead initial response to and investigate WHS incidents which is recognised under the Australian Qualifications Framework. This certification is nationally recognised. The Statement is issued by Right Choice Training (RTO 45790).

This is an open entry course with no pre-requisite qualifications. To ensure you complete the course successfully, the following entry requirements apply:

Participants must:

  • Be aged 18 years or over
  • Have recent minimum two years of industry experience
  • Have sound language and literacy skills (at least Year 10 English, or equivalent)
  • Able to commit to the course duration and time commitment
  • Learners must have access to a computer and the internet
  • Must supply a Unique Student Identifier (USI)
